During the wait period we highly encourage you to use our Home to Home re homing option. If you are able to re home prior to your appointment please call us to cancel.

We ask that you bring any previous pet veterinarian/vaccination records. Proof of ownership and a valid ID is required.
Is there paperwork I need to fill out?
The owner of the animal will be required to fill out an Owner Give Up Form that provides us information on your pet while he is in our care. In addition the owner will be required to sign a statement that relinquishes the pet to the custody of HSOY. The paperwork takes approximately 15-20 minutes to fill out so please allow enough time to complete the forms.
